DESCRIPTION
Warfarin sodium is used for the prevention and treatment of venous thrombosis (a blood clot that forms within a vein) and complications associated with atrial fibrillation and/or cardiac valve replacement.
The drug also is used to reduce the risk of death and thromboembolic events such as stroke or embolism following a heart attack.
Because the effects of warfarin are delayed, heparin is the anticoagulant of choice when an immediate effect is required. Coumadin generally is used for follow-up anticoagulant therapy after the effects of full-dose heparin therapy have been established and when long-term anticoagulant therapy is indicated.

Atrial fibrillation, prosthetic heart valve embolism prophylaxis, venous thromboembolism.
NEWLY DISCOVERED USES (OFF-LABEL)
Peripheral artery disease, stroke risk reduction, thrombectomy in peripheral artery disease, transient ischemic attack.
POTENTIAL SIDE EFFECTS
Bleeding, lethargy, malaise, asthenia, pain, headache, dizziness, rash, nausea, bruising.
CAUTIONS
- Do not switch brands once desired therapeutic response has been reached.
- Notify your doctor if you have active tuberculosis, diabetes, protein C and S deficiency, hemorrhagic tendencies, severe uncontrolled or malignant hypertension, severe liver disease, invasive procedures with potential for bleeding, risk of falls, pregnancy, warfarin-induced necrosis, ascorbic acid deficiency.
- Concurrent use with NSAIDs or aspirin may cause severe gastric irritation and increased risk of bleeding.
- Warfarin should be stopped three days prior to surgical procedures and blood monitoring checked prior to procedure.
DRUG INTERACTIONS
Warfarin interacts with many other drugs, the most important of which are listed below.
If this medication and the interacting drug are both taken regularly, the dosage of Coumadin will need to be adjusted to take this into account.
Short courses of interacting drugs may also affect the dose of warfarin needed.
- May reduce the anticoagulant effect of Coumadin
Acitretin, aminoglutethimide, rifampicin, antiepileptic drugs (carbamazepine, phenobarbitone (phenobarbitol), and primidone),
griseofulvin, oral contraceptives, vitamin K.
- May enhance the anticoagulant effect of warfarin
Anabolic steroids, analgesics (co-proxamol and paracetamol), anti-arrhythmic drugs (amiodarone and propafenone), valproate,
antifungal drugs (fluconazole, itraconazole, ketoconazole, and miconazole), disulfiram, ifosfamide, lipid-lowering drugs (clofibrate and simvastatin), proguanil, ritonavir, sulphinpyrazone (sulfinpyrazone), testosterone,
thyroxine, ulcer-healing drugs (cimetidine and omeprazole).
- Antibiotics: the anticoagulant effect of Coumadin is enhanced by cefamandole, chloramphenicol, ciprofloxacin, cotrimoxazole, erythromycin, metronidazole, oxfloxacin, and possibly by certain other antibiotics.
- May either reduce or enhance the anticoagulant effect of warfarin: Phenytoin, cholestyramine (colestyramine).
- Aspirin: increases the risk of excessive bleeding.
- Corticosteroids: may alter the anticoagulant effect of Coumadin.
- SSRIs, flutamide, tamoxifen, danazol: increases the anticoagulant effect of warfarin.
- NSAIDs: the anticoagulant effect of Coumadin is seriously enhanced by azapropazone and phenylbutazone.
- Diclofenac, difusinal, flurbiprofen, mefenamic acid, meloxicam, piroxicam, and sulindac can cause peptic ulcers, giving a site for possibly severe bleeding if these drugs are used with warfarin.
FOOD INTERACTIONS
Avoid or decrease quantities ingested of foods rich in vitamin K, vitamin E, cranberry juice.
HERBAL INTERACTIONS
St. John's wort, alfalfa, coenzyme Q10, cat's claw, dong quai, bromelains, evening primrose, feverfew, red clover, horse chestnut, garlic, green tea, ginseng, ginkgo, avocado, boldo.
PREGNANCY AND BREAST-FEEDING CAUTIONS
FDA Pregnancy Risk Category X. Do not take while pregnant. Consult your doctor regarding breast-feeding.
SPECIAL INFORMATION
It is important to not change dietary consumption of vitamin K once stabilized on warfarin therapy. Foods which contain vitamin K include, but are not limited to: beef liver, pork liver, green tea, leafy green vegetables.
Warfarin has been used in patients with peripheral arterial occlusive disease (a condition where the artery is narrowed). In patients undergoing thrombolectomy (a procedure done to remove clots from a blood vessel), it is recommended to use heparin followed by oral anticoagulation to prevent recurrent embolism.
Long-term oral anticoagulation with warfarin, with or without aspirin, is recommended by physicians in selected patients.
